Print it out (there is a Print button at the bottom of the form).
Mail the printed form, along with your check or money order for the full amount, to the following address.

CD's are $18.00 each.

Make the check out to "The Internationals".

Be sure to indicate which CD's you are ordering, and include your full name and address
so the CD's can be sent to you by return mail.
